Student Information Form - Must be completed at the beginning of the school year for every child/youth involved with the CFSA and each time the child/youth experiences a school placement change. This form provides critical information about the student, his/her educational decision-maker, and the child placing agency. Both the social worker and the social worker’s supervisor must review and sign this form affirming completion and accuracy of the information provided. The original document should be given to the school registrar at the time of enrollment.
